Aligning Population and Community Health To Advance Health Equity: Rush’s Response to COVID-19

The pandemic and recent uprisings have magnified the need for health care to be more responsive to the factors that contribute to inequitable outcomes in health and quality of life. This takes strategies that are attentive to the social risk factors and social needs of individuals, families, and communities – strategies that include both population health as well as community health initiatives that leverage interprofessional team members and social care activities. This webinar will highlight how teams at one institution - Rush University System for Health - have collaborated to anticipate and respond to the pandemic’s inequitable impact, with an emphasis on systems alignment and sustainability.
