CHaSCI is housed at Rush University System for Health, a large academic health system on the west side of Chicago. We are run by the Department of Social Work and Community Health.

+ Working in strong collaboration with others

CHaSCI's care models Bridge and AIMS have been developed collaboratively with partners from across Illinois and tested in diverse communities and systems. Our care model partners include aging network community-based organization Aging Care Connections and public health leader from Health & Medicine Policy Research Group.
Today, we collaborate actively with numerous local and national partners on care model research, policy advocacy, and communications development - including via the National Coalition on Care Coordination and the Coalition for Social Work and Health
