Systemic barriers impede the integration of health and social care.

This hinders our ability to provide effective care and exacerbates poor health and inequitable outcomes.

CHaSCI works to address these barriers by seeking governmental and institutional policy change with coalition-based advocacy and communications development.

 

National Coalition on Care Coordination

Since 2008, the National Coalition on Care Coordination (N3C) has helped advance public policies and programs that support coordinated and integrated care. CHaSCI provides convening leadership for N3C.

Coalition for Social Work and Health

The Coalition for Social Work and Health (CSWH) has been meeting since 2014 to elevate social work’s role and value in improving our nation’s health. CHaSCI provides convening leadership for CSWH.

Communications development

Part of policy change is building consensus among practitioners, stakeholders, and the public. CHaSCI regularly develops and contributes to strategic communications with this aim.
